  • Purchasing and fitting of prescription glasses, as well as repairs, or elective laser vision correction procedure, including contact lenses and prescription sunglasses, reimbursed at 80% to a maximum payment of $175 in any two (2) consecutive calendar years for adults, and once in any calendar year for eligible dependents under age 18, only with a prescription change, which has been recommended by an optometrist or ophthalmologist. However, if special eye glasses are required due to eye surgery performed while insured, up to $150 during a lifetime will be paid for frames, lenses, and the fitting of prescription glasses.
  • Contact lenses prescribed for severe corneal astigmatism, severe corneal scarring, keratoconus or aphasia, provided vision can be improved to at least the 20/40 level by contact lenses (but cannot be improved to that level by regular glasses). Payment will be made up to $250 per Insured Person in any two (2) consecutive calendar years.
  • Treatment of eye for accidental injury or disease.
  • Diagnostic services for suspected disease.
  • Services for visual training or remedial exercises at 50% co-insurance.
  • Eye examinations (including refraction) – one (1) per calendar year for dependent children and one (1) in any two (2) consecutive calendar years for other persons.
  • Laser eye surgery, with a lifetime maximum of $500 per insured member.
